ABSTRACT

The etiology of slipped capital femoral epiphysis (SCFE) is unknown, though hormonal as well as mechanical factors have been implicated. We report a case of gigantism who presented with SCFE. This case provides an insight into the genesis of SCFE, which in this case was related to growth hormone excess and sex-hormone deficiency.

ABSTRACT

Slipped capital femoral epiphysis is rare in Asiatic Indonesian-Malays. Seven cases (9 hips) of this condition in Ramathibodi Hospital including five boys (average age, 12.5 years) and two girls (average age, 13 years) were reviewed. Most of the cases (4 out of 7) were acute on chronic and mild slips. No endocrine disorder was observed in all cases. All of the patients had a body weight above the mean of the normal population, four of which were obese. For the treatment, a single screw fixation including one case with cancellous and six cases with cannulated type were used. In the follow-up of average 2.5 years, six cases had satisfactory results. Avascular necrosis occurred in one case with mild and chronic slips in which a cancellous screw was used. It is concluded that obesity is the important factor related to the etiology in this study and probably is the same in other developing countries. The effect of a cancellous screw causing avascular necrosis is still questionable.

ABSTRACT

Una niña de 8 meses de edad ingresó al hospital por crisis convulsivas tónico clónicas generalizadas, sopor, irritabilidad, hemorragias retinales y disyunción epifisiaria de los huesos. La lactante era frecuentemente sacudida por su madre -soltera, adolescente- entre los brazos de ésta, con el objeto de silenciar su llanto. En el examen de los fondos de ojo había atrofia de ambas papilas ópticas y en la tomografía axial del cráneo se comprobó un derrame subdural frontal bilateral que obligó a drenarlo quirúrgicamente. A la edad de 2 años y 3 meses la paciente tiene retardo psicomotor severo y ceguera bilateral. El síndrome del lactante sacudido es una de las formas mas graves de maltrato físico infantil y es difícil de reconocer por cuanto suele no presentar manifestaciones externas de la agresión, por lo que es importante tenerlo presente

ABSTRACT

O estudo retrospectivo de 273 pacientes submetidos a transplante renal, no período compreendido entre julho de 1973 e novembro de 1989, revelou apenas 12 casos com manifestaçöes osteo-articulares, dada a reduzida prevalência de osteonecrose, justificada pela utilizaçäo de baixas doses de corticosteróides. Levando-se em conta que dois pacientes apresentaram mais de uma manifestaçäo, tem-se: quatro casos de artrite infecciosa; cinco com artralgias pós-transplante; três osteonecroses; uma artrite otosa e uma epifisiólise da cabeça femoral
